Bridge the Gap was set up in August 2014 as a charity to support and improve emotional care to young adult patients with long term health needs throughout the teenage years and in the difficult transition to adult medical care.Up to the age of 16 a young person receives care in a childrens ward with their care transferred to adult services at 16 with inpatient care in an adult ward,where support levels are very different ,yet a teenager does not become an adult overnight!
